Setting up power z. Problems with wiring

I'm trying to install a power z table tinto a kt-3020 but i'm not sure of the connections. I have the SC-04 and an M415b driver. Does anyone have a schematic or could you tell me the connections from teh sc04 to the driver? The pulse and direction match up but i'm not sure which other wires to connect.

As matter of fact, I did created the diagram and posted it a while ago:

2M415 or M415B are the same from different provider
"Opto" = +5V
