5v power regulator 4812s6

I just want to make sure I am understanding how to correctly wire this regulator to monitor 36v from a voltmeter. See the attached image to view my interpretation. The wiring for the regulator doesn't seem straight forward to me (especially the need to hook up 2+ and 2- on the Volts in), this is the reason that I need advice. Thanks
