When I look at the unit -
The wire colors from Left to Right is: Orange, Black, Green, Black
Above the plug on the circuit board, is the following:
INDC-60V DC12V
+ --- --- +
If I assign pin numbers from left to right, 1-4,
Is correct wiring?
Pin 1 - Positive Input Voltage
Pin 2 - Negative Input Voltage
Pin 3 - Negative Output Voltage
Pin 4 - Positive Output Voltage
If so, I would think that the wires on Pins 3 and 4 would both be black.
Is there a hookup diagram somewhere?

Yes, it's correct. If you look at the board closely, you can see "+0-60V - - 12V+"
It's common ground, so the middle two pins are (-)

I think there is a design problem on this module. It seems that my supplier didn't do it right and the module get very hot when input is over 40V and currrent is over 200mA. Instead of using a coil (L) 330uF, they put a resistor instead. And the resistor is getting very hot as well.
